Family Nurse Practitioner Joins Central Carolina Community Family Care

Baylee Smith, FNP-C, Family Nurse Practitioner, has joined Central Carolina Community Family Care and is seeing patients in Sanford as of April 21.
She joins the practice of Daphne Cates, MD, and Amie Motley, FNP-C, at 2412 Wilkins Drive in Sanford.
“We are pleased to welcome Baylee to Sanford and Lee County,” said Dave Santoemma, CEO of CCH. “Her presence will help meet our community’s growing primary healthcare needs.”
A native of Temecula, CA, Smith received her Associates Degree in Nursing from Butler Community College in El Dorado, KS, in 2019. A year later, she completed her Bachelor of Science in Nursing degree from Southwestern College in Windfield, KS. In 2024, she earned her Master of Science in Nursing degree from the University of Southern California in Los Angeles.
Smith completed her clinical rotations as a Student Family Nurse Practitioner at Goshen Medical Center in Clinton, NC, in 2023 and 2024. She also worked as a Student Family Nurse Practitioner at Cal Oaks Primary Care in Murrietta, CA, in 2024.
Prior to that, she worked as a Registered Nurse at Tripler Army Medical Center in Honolulu, HI, in 2021-22, as a Registered Nurse at Central Carolina Wound Care and Hyperbaric Medicine in 2023 and as a Registered Nurse Case Manager at Advanced Home Health in San Diego, CA, in 2023-24.
Smith will see patients of all ages. Her clinical interests include women’s health, orthopedics, chronic care management, lifestyle medicine and patient education.
Outside work, Smith enjoys reading, exercising and watching her children play sports on the weekends. She plans to live in Carthage.
