Your Patient Portal has the most up to date records and can be seen easily once you set up your account.

You can request your medical records online, by fax or in person.

To fax your request, print out the authorization Medical Records Authorization for Release of Information.pdf, fill it out and fax to 877.792.5632.

To pick up records in person, visit the Health Information Management (Medical Records) Department, located in the Medical Arts Building near the front lobby of the hospital.
