iThe third floor of CCH is a medical/telemetry unit providing care to inpatients and observation patients. Patients are 18 years and older with a primary focus on geriatric patients. Primary diagnoses are:
RN's, LPN's and Nursing Assistants provide care. Charge Nurses on each shift make assignments with a focus on safety, acuity level, and quality. The Nursing Director has 24-hour responsibility for the unit.
Patients are admitted to the hospital through their physician's office as a direct admit or through the Emergency Department to the Hospitalist Program. All admissions are admitted through the Registration department. A picture ID and insurance information needs to be brought when registering, as well as any advance directive information, such as a living will and/or health care power of attorney.
